Our Priority Population
Our priority populations include individuals who are unattached without a primary care provider and from one or more of the following groups:
Individuals who are from racialized groups or communities
Individuals with physical or mental disabilities, including mental health conditions and/or substance use challenges
Individuals who identify as part of the 2SLGBTQ+ community
Individuals experiencing homelessness
Newcomers, including immigrants and refugees
Individuals living in geographically or physically isolated areas who face barriers to accessing care
Individuals who self-identify as Indigenous
Individuals living in conditions of extreme poverty
Embracing Reconciliation
in Community Healthcare
The Peterborough CHC is committed to a reconciliation model of healthcare, serving both Indigenous and non-Indigenous peoples. Drawing inspiration from the Wiingashk, Sweetgrass braid, the Peterborough CHC weaves together the strengths of our diverse communities. Each strand of the braid symbolizes an essential aspect of reconciliation: Indigenous knowledge, Western practices, and the collective community wisdom that comes together to share space.
Just as each strand retains its unique characteristics while contributing to the strength and integrity of the braid, reconciliation respects and values the distinctiveness of all peoples. The act of braiding itself is deliberate and collaborative, much like the process of reconciliation, which requires intentional effort, mutual respect, and a commitment to healing. By envisioning reconciliation as a Wiingashk, Sweetgrass braid, we emphasize the importance of weaving together our diverse stories, traditions, and strengths to create a healthy and resilient community for future generations to come.

The CHC Model of
Health and Wellbeing
is a comprehensive, inclusive, and holistic approach to healthcare, addressing both medical and social needs. CHCs provide accessible, client-centered care through interdisciplinary teams, offering a wide range of services including primary care, illness prevention, and health promotion. CHCs eliminate barriers to care and focus on social determinants of health through support services and advocacy. Governed by community boards, CHCs ensure services are responsive to community needs.

The Indigenous Model
of Health and Wellbeing
is grounded in a population needs-based approach to healthcare planning and delivery for the Indigenous Peoples. The Peterborough CHC employs an Indigenous and community-centered holistic approach to enhance the mental, emotional, physical, and spiritual well-being of Indigenous Peoples in our communities. Traditional knowledge, healing practices and approaches, and self- determination are essential in restoring balance at the individual, familial, and community level. We honour that culture is treatment and culture is healing.
